چکیده

We report for the first time two cases of vein graft spasm following distal artery bypass in the lower extremity. In the first case, graft spasm occurred in the region just distal to the proximal anastomosis and it resolved without surgical intervention. In the second case, completion angiography showed defective filling around the distal anastomosis. We considered this to be due to technical error, and created another bypass to avoid the stenotic area. However, postoperative angiography revealed that the defective filling was due to graft spasm. Native artery spasm has been reported previously, but graft spasm has not. However, we suggest that graft spasm could be more frequent than previously recognized, and could result in immediate thrombosis of the graft. (J. Jpn. Coll. Angiol., 2003; 43: 64-66)

Aortocoronary vein graft spasm, a clinical entity?

T he concept that saphenous veins which have been transplanted into the arterial circuit are passive, unreactive conduits may be an oversimplification. The unique report by Victor et al in this issue (see page 413) proves that such vein grafts can go into severe spasm. Their report describes a patient who suffered recurrent angina pectoris after undergoing a triple coronary artery bypass operat...

Mega-aneurysmal degeneration of a saphenous vein graft following infrainguinal bypass. A case report.

True aneurysm formation in infrainguinal autologous vein grafts is rare. The authors report a case of a patient presenting with extensive aneurysmal degeneration of a femoropopliteal bypass vein graft 13 years after the original operation and 8 years after graft revision and thrombolysis for acute occlusion. Over a 2-year period, the vein graft aneurysm expanded from 2.7 to 4.5 cm maximal diame...

Treatment of failing vein grafts in patients who underwent lower extremity arterial bypass

PURPOSE We attempted to determine risk factors for the development of failing vein graft and optimal treatment in patients with infrainguinal vein grafts. METHODS We retrospectively reviewed a database of patients who underwent infrainguinal bypass using autogenous vein grafts due to chronic atherosclerotic arterial occlusive disease of lower extremity (LE) at a single institute between Septe...

HYPOTHESIS Percutaneous angioplasty would provide a durable alternative to surgical revision in the treatment of infrainguinal vein graft stenosis. DESIGN Outcome analysis of the results of percutaneous angioplasty of infrainguinal vein graft stenosis. SETTING Academic vascular surgical practice in a university-affiliated community hospital. PARTICIPANTS All patients undergoing percutaneo...

Remodeling leads to distinctly more intimal hyperplasia in coronary than in infrainguinal vein grafts.

BACKGROUND Flow patterns and shear forces in native coronary arteries are more protective against neointimal hyperplasia than those in femoral arteries. Yet, the caliber mismatch with their target arteries makes coronary artery bypass grafts more likely to encounter intimal hyperplasia than their infrainguinal counterparts due to the resultant slow flow velocity and decreased wall stress. To al...
